Rachel’s journey began as a hands-on Skin Therapist, a role that has taken her around the world, accumulating invaluable experience and accolades, including New Zealand Therapist of the Year. Her dedication to skin health and her expertise in corneotherapy have made her a leading voice in the field, earning her a place on the Education Board for the International Association of Corneotherapy.
 
During our conversation, Rachel delved into the science behind corneotherapy, the drawbacks of chemical peels, and the importance of holistic skincare that includes both internal and external care.

What is Corneotherapy?

Corneotherapy is a revolutionary approach to skincare focused on repairing and maintaining the skin barrier. Unlike traditional treatments that can be harsh and invasive, corneotherapy works from the outside in, preserving the epidermis and supporting the skin’s natural defenses. This approach ensures that the skin’s defense and immune systems are not unnecessarily stimulated, maintaining overall skin health.

Anneli:

Strong chemical peels, like Cosmelan or Jessner, are often used for quick results in treating certain skin conditions. However, the idea of peeling off layers of beautiful skin in one go sounds horrific to me. Beyond removing the skin barrier, what else happens at a cellular level when using these strong peels?

Rachel:

Some clients may see short-term improvements in pigmentation or acne, but these peels don’t address the root cause. They only temporarily diminish the appearance of these issues. The problem with such deep peeling is the long-term damage it can cause to essential cells deeper down, like those involved in the mitosis process for Keratinocytes.
 
Keratinocytes need to be healthy to communicate effectively with other cells. When you peel at the dermal/epidermal junction level, as with a Jessner peel, you destroy Keratinocytes. The subsequent mitosis process then creates damaged Keratinocytes, leading to cellular aging and incorrect communication with other cells.

Chemical peels, unlike proteolytic enzymes, are keratolytic, meaning they loosen the bonds that hold skin cells in place. These peels involve applying a chemical compound to the skin, often more acidic than the skin’s natural barrier. Some, like Trichloroacetic acid (TCA), can be corrosive if not used with expertise.

Chemical peels are categorized by their depth of penetration:
Superficial depth peels
Remove epidermal layers without going beyond the basal layer.
Medium depth peels
Penetrate the epidermis and reach the upper dermal layers.
Deep peels
Reach the mid-reticular dermis.

Peels not only remove the skin's first three lines of defense but are also typically synthetic and incompatible with the skin’s metabolic functions. Some peels can be comedolytic, which exacerbates the destruction of intercellular lipids, leading to post-treatment dryness and a slow recovery of the barrier defense system. Given their potential to disrupt cellular connections and communication, most chemical peels are considered more destructive than beneficial.

Anneli: 

Is healthy food and supplements enough for healthy skin?

RACHEL:

While nutrition is crucial for skin health, our internal organs absorb most of what we ingest, often leaving little for the skin. Our skin, being the largest organ, needs a lot of nutrients. A balanced internal microbiome positively affects the skin, but external environmental stressors also play a significant role. That’s where topical skincare comes in, providing protection from the environment.
 
Without a healthy diet, your skin won’t look good. Skin needs to be treated holistically, considering all lifestyle choices. There’s no magic bullet; it’s a combination of good nutrition, skincare, and overall healthy living.

Anneli: 

Skin is a reflection of our health. A lot of our internal issues are reflected on the skin, which we need to treat systemically most likely, but what benefits do we have if we treat skin topically?

Rachel:

The gut microbiome is as important as the skin’s microbiome, and both need protection. Using the right topical skincare helps maintain the skin’s barrier defense system. When this barrier is compromised, pathogens can trigger the immune system, leading to persistent inflammatory responses.
 
For example, with acne, an internal trigger combined with an imbalanced skin microbiome can allow acne bacteria to penetrate deeper, causing problems. Treating skin topically helps prevent scarring and post-inflammatory pigmentation, addressing both symptoms and root causes.
 
Corneotherapy is all about prevention. Genetics play a role, but environmental stressors mean everyone should use topical skincare. Post-COVID, our immune system response has changed, with more people experiencing skin issues like dermatitis, rosacea, and psoriasis. Strengthening both the gut microbiome and the skin barrier is crucial.

Anneli: 

Some believe that during the night time we should let our skin rest and apply only some light textured products. What are your thoughts on night time skincare?

Rachel:  

Nighttime is when your body and cells are repairing themselves. By feeding your cells with what they need, you enhance this process. It’s not so much about the consistency—whether it’s a cream, serum, oil, or gel—but about the ingredients they contain and the benefits they provide to replenish and restore the cells. My absolute essentials are Vitamin A, which helps with DNA repair, and Vitamin C, which supports overall cell health and provides cellular energy for repair.
 
It also depends on what skin type people have. Are they naturally or genetically lipid-dry? Do they have a balanced skin barrier to protect and repair itself? If the barrier is impaired, your skin might spend the night in a fight-or-flight state, triggered by an inflammatory response.
 
Inflammatory skin conditions can be triggered by heat, and our body temperature is usually higher during the night (under the duvet, etc.), so the more we can keep the barrier protected and balanced, the better.
